AWARD APPLICATION

Purpose of Award: To recognize an exceptional level of Scout, parent and leader participation in District and Troop activities.

Eligibility: First 4 criteria are mandatory. Completion of a total of 8 criteria for the 'silver' award and 10 for the 'gold' award.

Due date: January 11, 2000 (January Roundtable)

______	1. Leader Training: Scoutmaster has completed Scoutmaster Fundamentals Basic Training
______	2. Troop Awards: Troop received Quality Unit Award.
______	3. Parental Participation: Troop has an active and working committee
______	4. District Fund Raising: Troop participated in two activities that provide financial support for the District such as "Friends of Scouting" or Popcorn Sales. List activities: a. ___________________________ b. ___________________________

______	11. Assist in District Leadership: Provide 'volunteer' help in a District-wide activity (e.g., Merit Badge Pow Wow, First-Aid Contest, day camp) or supply a trainer for Scout Basic or Pow Wow Training. List activity: a. ___________________________

Foot notes: The same activity cannot be used to meet two different criteria, e.g., Scout Show ticket sales can be used as either a Scout Show activity or a District Fund Raising activity, but not both. "Participation" means all eligible Scouts in the Troop were invited to participate and at least one Scout actually did. The patch order will be finalized immediately after the January Roundtable in hopes of having the patches available for the District Recognition Banquet. The patches will be the existing design with the year added. The border color will indicate silver or gold award.
